Do you need Northwoods Home Health Services?

If you or a loved one is recovering from an injury,  illness, or is disabled, chronically or terminally ill and is in need of skilled services, Northwoods Home Health is committed to providing quality, personalized in-home care. Northwoods is dedicated to providing a wide variety of clinical, social and supportive services to people of all ages. We provide both medical and non-medical assisted living services and hospice support. We realize that each person’s care is unique and we rely on your healthcare professionals to decide which care will be best for you.

We provide an array of home health medical and personal care services, including:

  • Nurses — Provide assessment, instruction and help with proper healing, disease and medication management, and pain and symptom control.
  • Physical, Occupational & Speech Therapists — Develop a coordinated rehabilitation plan based on your needs to safely promote and enhance mobility and daily activities.
  • Patient Care Coordinators/Social Workers —
  • Evaluate the social and emotional factors affecting individuals with illness and disabilities. Coordinate services for patients who require both medical and ongoing or long-term care.
  • Home Health Aides — Help patients with activities of daily living, such as getting in and out of bed, walking, bathing, toileting and dressing. Under the supervision of a clinician, aides can provide more specialized care such as exercise routines and simple dressings.
  • Homemakers — Perform light household duties such as laundry, meal preparation, general housekeeping and shopping. Homemakers also provide friendship and emotional support for many people who live alone.

Who Pays for Home Health Care?

Medicare, Medicaid, private insurance and private pay sources cover or partially cover home care services. Eligibility and coverage for home healthcare services differs for each insurer and many have various plans and benefits for different groups. Your individual plan may have deductibles, a co-pay or limits. If you do not have home healthcare coverage, we will work with you to determine appropriate available services.

Remote Patient Monitoring Offers Peace of Mind

In order to deliver the highest quality care to our in-home patients, Northwoods Home Health offers patients and their medical caregivers the latest technology to monitor patients at home. Called telehealth, this simple technology allows patients to play an active role in their own health by taking their vital signs daily. Vital signs, including weight, blood pressure, pulse, glucose levels, oxygen levels and others, are taken using small, wireless devices placed in the home. The results are automatically transmitted to a web-based server through an in-home communication device called a TeleStation. This information keeps clinicians informed of immediate patient needs and identifies high-risk patients for early intervention.
